New Path Coaching, LLC - Client Informed Consent

COACHING: is a dynamic growth-oriented process to help you address issues and concerns related to understanding yourself and how you choose to engage the world around you. This involves a relationship between you and a trained life coach who has the desire and willingness to help you accomplish your individual goals. The outcome of coaching is often positive, however the level of satisfaction for any individual is not predictable. Your life coach is available to support you throughout the coaching process. 

Coaching is only intended for individuals with non-clinical levels of distress. Coaching is not counseling. As Tammy Pacheco, MA, LPCC is a licensed professional clinical counselor, she may determine during the course of coaching that clinical treatment may be necessary. She is prepared to refer you to a licensed mental health professional if warranted.

CONFIDENTIALITY: All interactions with Tammy Pacheco, MA, LPCC and New Path Coaching, LLC, including scheduling of or attendance at appointments, content of your sessions, and your records are confidential. However, because of licensing requirements, she is required to report the following exceptions, even though she is not engaging in a counseling role.

EXCEPTIONS TO CONFIDENTIALITY

• If there is evidence of clear and imminent danger of harm to self and/or others, a therapist is legally required to report this information to the authorities responsible for ensuring safety.
   
• Ohio state law requires that any therapist who learns of, or strongly suspects, physical or sexual abuse or neglect of any person under 18 years of age must report this information to county child protection services.

SCHEDULING: I appreciate prompt attendance of sessions. Please notify me at 513-226-5648, if you will be late. Twenty-four hour notice of cancellation allows me to use the time for others.

I have read and discussed the above information with my coach. I understand the risks and benefits of coaching, the nature and limits of confidentiality, and what is expected of me as a client of Tammy Pacheco, MA, LPCC and New Path Coaching, LLC.
